SSM Health is actively scheduling interviews with RNs for our various facilities across St.

Louis.

We are looking for Registered Nurses with a wide range of backgrounds and specialties.Various work shifts are available with full-time and part-time scheduling options in the following areas: Behavioral Health, Ambulatory Surgery, Telemetry, Med Surg, Critical Care, ICU, and Endoscopy.Since 1872 SSM Health has been serving our community’s comprehensive health needs across the Midwest through a robust and fully integrated health care delivery system.

Identifies root cause for readmission.

Develops plans and collaborates with the multidisciplinary team regarding discharge planning to the next level of care.Collects comprehensive data pertinent to the health care consumers health and/or situation.Triages each health care consumer utilizing age, developmentally appropriate, and culturally sensitive practices to prioritize and optimize health care consumer flow, expediting those health care consumers who require immediate care.Participates with other members of the team regarding opportunities for improvement in standard work.May also be responsible for utilization management process, ED case management process, admission process, and act as a case manager liaison with post-acute providers (skilled nursing facilities, rehab, home health, etc.).Applies the existing body of evidence-based practice and scientific knowledge in health care to nursing practice, ensuring that nursing care is delivered based on patient’s age-specific needs and clinical needs as described in the department’s Scope of Service.Demonstrates the professional nursing standards defined in the professional practice model.

Uses the ANA Code of Ethics for Nurses to guide his/her response to the current and evolving health and nursing needs of our patients and our patient populations.Works in a constant state of alertness and safe manner.Performs other duties as assigned.EDUCATION Graduate of an accredited school of nursingEXPERIENCE No experience necessary.
